Forever Young COVID-19 Wellness Practices Checklist (as of December 2020)

Please carefully review and sign this checklist, as our ability to remain open depends on the vigilance of every family.

With the number of COVID-19 cases rapidly increasing in California, we want to reiterate some best practices and standards. We ask that all parents/caregivers do the following:

  • Wear a cloth face covering or mask prior to entering Forever Young.
  • Do not go past the cubby area.
  • Wash/sanitize hands before entering the premises.
  • Do not leave your child until the temperature check is completed. If your child has a fever of 100.4 or higher, they will need to stay home for two (2) weeks and return to Forever Young only with a physician’s note.
  • If there is another parent/caregiver dropping off or picking up their child, please wait outside the door.

If you are showing symptoms for COVID-19 or have come in close contact with someone who has tested positive for COVID-19, please let Selina or Carl know as soon as possible. Any of the following counts as “close contact”:

  • You were within 6 feet of the person for a total of 15 minutes or more.
  • You provided care at home to the person.
  • You had direct physical contact with the person (hugged or kissed them).
  • You shared eating or drinking utensils.
  • They sneezed, coughed, or somehow got respiratory droplets on you.

If any of the above applies to you, we ask that you get tested as well as keep your child home from Forever Young for the next two (2) weeks.

Please remember that people infected with COVID-19 can exhibit a wide range of symptoms— from mild symptoms to severe illness. And these symptoms can arise any time between 2 to 14 days after exposure to the virus. Possible COVID-19 symptoms include:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

Also be aware of emergency warning signs for COVID-19. If someone is showing any of these signs, please seek emergency medical care immediately:

  • Trouble breathing
  • Persistent pain or pressure in the chest
  • New confusion
  • Inability to wake or stay awake
  • Bluish lips or face

If COVID-19 is confirmed in any child, household member, or staff member, Forever Young will need to close for approximately two weeks. Staff will take all necessary measures and extra precautions by disinfecting the building according to the guidelines established by the California Department of Social Services (CDSS) Community Care Licensing Division, the Centers for Disease Control and Prevention (CDC), and the California Department of Public Health.

Disinfecting and keeping everyone safe are our main priorities. The Forever Young staff have scheduled times to sanitize toys, playground equipment, all fixtures, the restroom, and other materials in use. We will continue to take these precautions until it has been deemed safe.

Free COVID-19 tests are available through Sacramento County and other services (e.g., Project Baseline).

I have read and understand the COVID-19 Wellness Practices Checklist. I vow to do my part in keeping the children and staff members of Forever Young as safe as possible during this time.
